I certify that the information I have provided on this consultation form is correct and that I have been fully informed of the importance of using recommended hair extension aftercare products to care for my hair extensions. I understand that correct aftercare products are essential & I am responsible for making sure these products are used during the entire period I am wearing hair extensions. I also agree to carry out all advice given to me by my hair extension technician. I will not hold the hair extension technician responsible for any damage or injury caused by me failing to adhere to the correct aftercare advice, or for the condition of the hair extension hair. I understand the importance of regular maintenance appointments to keep both the health of my own hair, and the extension hair, at their best.
